Community Connect (Coordinate Intake & Referral)
What is it?
Healthy Start’s Community Connect (CI&R), assists pregnant women, new mothers, and their infants by providing a one-stop entry point for needed services and programs. Our programs are dedicated to ensuring pregnant women, new mothers and their infants have access to care, pregnancy and parenting education, child development education, breastfeeding education, and support services. Community Connect works to identify a pregnat woman, new mother and their infants needs and links them with local programs that can best serve them, creating a referral process that not only streamlines access to services but works to build a larger network of care during pregnancy, post partum, infant birth, early childhood.

Who is Eligible?
Any female resident of Manatee County who is pregnant and at a risk for a poor pregnancy outcome, including children up to age three, as well as women who are between pregnancies. Community and self-referrals are also accepted. Participation is free and voluntary for women, regardless of age and marital status.
